Our Fees

  • FEES FOR COUNSELING

    Fees vary depending on many things including insurance rates. Cash rates are available and vary by counselor. Please reach out to us for additional information.

  • FEES FOR SERVICES OTHER THAN COUNSELING

    Health insurance plans including private insurance, Medicaid, Medicare, and Tricare do not cover the costs related to these services and are therefore the financial responsibility of the client.

  • Documentation Fee – $150.00 per hour, 30 minute minimum

    This fee is to compensate your therapist for the time invested in completing forms on your behalf (e.g. FMLA, disability, school accommodation requests).

    Any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms have a fee of $150.00 per hour.

    FMLA paperwork generally requires a minimum of 60 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete and may require additional assessments beyond a regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to lawyers or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $150.00, with a minimum fee of $75.

    Your therapist will discuss minimum session requirements with you (often 6-8 sessions) before completing any FMLA, disability, other paperwork or letters of support.  Your therapist will also not complete any FMLA or disability paperwork if they do not believe they can support it based on what you have presented at intake and during sessions.

    $75 fee must be paid prior to therapist completing paperwork. Remaining balance due upon receipt of documents.

  • Court Testimony or Appearance Fees: Please inquire with your individual therapist

    In the event that it is legally required by court order/subpoena to participate in any legal proceedings, a fee will be assessed to accomodate our time for:

    • Preparation and submission of records

    • Consultations with other professionals involved in the case

    • Travel time to/from court-related activities

    • Any other court-related services provided

    Your therapist will be required to clear their calendar of all appointments and prior engagements so that they may be available to appear in court. Therefore, this fee is non-refundable.  If your court date is postponed and your therapist must again clear their calendar to attend court, you will again be charged the full fee.  If your court date is cancelled this fee is non-refundable. 

  • Please note that we have counselors in our office that have set their own fees for services, and do not follow fees set by Burns Lilly Mending Minds. If you require any non-clinical services, please speak with your individual counselor regarding fees.

  • Records Request

    The fee for requesting medical records is $25 for the first 20 pages and $0.50 per page for subsequent pages in paper format. For electronic records, the maximum is $25 for up to 500 pages and $50 for more than 500 pages.

    Fee structure

    Paper format: Up to $25 for the first 20 pages, plus $0.50 for each additional page. 

    Electronic format: Up to $25 for 500 pages or fewer, and $50 for more than 500 pages. 

    Hybrid format: A combination of the paper and electronic fees. 

    Affidavit: A fee of $15 will be charged if an affidavit is requested to certify the records
